Help at Home by Montefiore is a unique service that is one of Sydney’s greatest offerings to clients that require support so they may maintain independence in their own home for as long as possible. Our team of carers provide around the clock care to each individual client and work closely with our dedicated team to ensure the "right" care is supporting the client.
Due to internal movement, we are looking for an experienced NDIS Support Coordinator to join our collaborative environment, this is a remote role where you will be on the road working with our NDIS participants across Greater Sydney.
This is a fantastic opportunity to work with participants to maximise the value of their NDIS plans, build capacity, and connect them with services that support their goals and independence.
The Opportunity
In this role, you will work closely with NDIS participants, their families, and providers to ensure supports are tailored, effective, and aligned to individual goals.
Your responsibilities will include:
Supporting participants to understand and implement their NDIS plans
Connecting participants with appropriate providers and services
Building participant capacity to manage supports independently
Monitoring budgets and ensuring effective use of funding
Preparing reports and supporting plan reviews
Liaising with families, carers, and stakeholders to coordinate supports
Maintaining accurate records and documentation
About You
To be successful for this opportunity you must:
Have experience in Support Coordination, Case Management, or within the Disability / Community sector
Have a strong understanding of the NDIS framework, plans, and funding
Have excellent written and verbal communication skills
Be highly organised and able to manage a varied case load
Have a can-do, client focused attitude
Pick up systems quickly
Have strong problem-solving skills and be results driven
Hold a current driver’s licence and have access to a reliable vehicle
Have a current NDIS Worker Screening Check
